Step 1. Complete COC
Complete the Chain of Custody (COC) document—it's essential for fast sample inventory. We can't accept your samples without it. (Jan 2025)

Step 2. Commercial Invoice
Complete and include a printed commercial invoice with the shipment for efficient customs clearance. (Jan 2025)

Step 3. Certification Statement
Complete and include a printed certification statement with the shipment to provide information about shipped items to customs. (Jan 2025)

Step 4. Import License
Print and include the import license with your shipment to prevent customs delays. (Exp. April 2025)

Step 5. Declaration (optional)
If your shipment is over 3 kg or urgent, we recommend declaring it with US Customs 72 hours before it arrives in the US. Our company can handle the declaration on your behalf if you complete and submit this form. (Feb 2025)
